ABSTRACT

The depression is the subject of a conceptual bursting, and authors propose a dichotomy "unipolar/bipolar" depression, with sometimes unclear limits. Moreover, within the group of the bipolar depression several under groups were identified, with revolutionary therapeutic repercussions. In this article, we propose to: point out the bipolar spectrum - Give a progress report on the incurred risks of the prescription of the antidepressants for a depression belonging to the bipolar spectrum - Propose therapeutic recommendations, while referring to a recent literature

ABSTRACT

Psychological factors may trigger or exacerbate symptoms of irritable bowel syndrome. This study was conducted to analyse psychological factors and personality of patients consulting for irritable bowel syndrome. 100 outpatients at Hdi Chaker hospital of Sfax who met Rome II diagnosis for irritable bowel syndrome, were scheduled for psychological and medical assessment, During psychological assessment, patients completed the Minnesota Multiphasic Personnality Inventory. Personality disorder was found in 68%of subjects [hysteric 32%, depressive 29%, somatization 29%, paranoiac 10%]. The study of psychologic profile of patients with irritable bowel syndrome has shown that psychologic distress is more frequently encountered in patients with abnormal personality than those with normal personality. Patients with irritable bowel syndrome have an increased prevalence of personality disorders. Psychological factors have a profound influence on the onset, expression and course of Stable bowel syndrome, particularly in more severely affected patients. Considerations should be given to a psychological disturbace, precipitating factors such as a major life stress, and assessment of personality disorders. A strong physician-patient relationship provide firm a psychological support

ABSTRACT

In addition to their common embryonic origin, the psychism and skin maintain privileged relations on several accounts. These relations can be found disturbed particularly among old people whose brain as well as skin have undergone the effects of senility. Ekbom syndrome or delusional parasitosis shows dermato-psychological intrications especially among the elderly. Through three clinical observations, the authors remind the clinical signs of this syndrome and discuss its psychopathological elements and its therapy. This syndrome is probably more frequent that described. To treat it adequately, it's important to situate it in nosographic context. This goes above all through a collaboration of the different involved disciplines

ABSTRACT

Gluco-corticoids are known since 1935. They are of interest in all medical fields. In psychiatric practice, the gluco-corticoids are arising our interest in two ways at least i.e: 1] psychiatric troubles secondary to corticoth‚rapy: this means no specific anxiodepressive features sometimes, but the most frequently met is hypomanic clinical feature, whose intensity is depending to own predispositions. In few cases, that means serious psychotic complication including often a confusion mark. 2] The Dexamethasone test [D.S.T]: this test is recently preconised as biological marker of endogenous depression: inhibiting secretion of cortisol is failing in about 95%. It was assumed that it has high specificity but after first enthusiasm its diagnosis value was criticized and its specificity was devaluated. That features give to psychiatric corticoid connexions a relative value but a very interest remains to such a connexion as a field of further research in biological psychiatry
